SUSPENSION OF PURE ACCOUNT

I would like to suspend my account effective the selected date below:
Note: Your requested suspension start date must be within 30 days of the date that your request is made.
For requests more than 30 days ahead, please log back on and submit it at a later date.

MEDICAL REASONS – I would like to suspend my account with immediate effect for medical reasons.
PAID SUSPENSION - I agree to and understand that a suspension fee of HK$288 per month applies. Whilst my account is suspended for any reason, I understand I will not be able to use PURE facilities and/or equipment and/or to take part in any PURE activity during the suspension period including access to PURECAST live-streamed classes, unless I reactivate my account.
IMPORTANT
  1. If you hold a Monthly Autopay Package, you may suspend your Agreement by completing a Suspension Request on or before:
    (i) the 5th day of the month if your Autopay date is on the 16th day of each monthly billing cycle;
    (ii) the 20th day of the previous month if your Autopay date is on the 1st day of each monthly billing cycle.
  2. If you have a Prepaid Agreement, you agree to pay the applicable suspension fee. Once payment is successfully collected and PURE confirms your suspension request, your suspension will be effective and PURE will adjust the expiry date of your Agreement.
  3. If you choose Pay by Link, PURE will send you payment instructions as soon as possible. Once payment is successfully collected, PURE will process your suspension request. If PURE fails to collect such payment, the suspension request will be cancelled.
  4. All fees are non-refundable and non-transferable. PURE reserves the right to reject your suspension request if:
    • The request is for backdated suspension;
    • The request does not comply with the valid suspension period (minimum 1 month, maximum 3 months);
    • You have already reached your suspension quota for the year (maximum 3 months unless authorised by PURE);
    • Your payment of the applicable suspension fee is not successful.
  5. PURE will confirm your suspension request by email as soon as possible of receiving this form.

Additional Terms and Conditions:
  1. You may pay to suspend your Cardholder Agreement from today’s date for a maximum period of up to 3 months in any calendar year for any reason or for a maximum of 12 months if you are pregnant. No backdated suspension is allowed.
  2. You agree to pay a Suspension Fee (currently HK$288 per month but this may be increased) during the suspension period.
  3. Unless authorised by PURE, suspension of your Cardholder Agreement must be for a minimum of 1 month and a maximum of 3 months. No suspension for part of a month is allowed.
  4. If applying for a medical or pregnancy suspension, you are required to provide a valid medical certificate that contains the date, your name, diagnosis, and the doctor's advice. PURE reserves every right to reject your suspension if such medical certificate is invalid and to charge you HK$288 per month.
  5. If you have rented a Monthly Locker from us, we will continue to debit your Autopay Account for payment of Monthly Locker Fees during the Proposed Suspension Period.
  6. If you have a Monthly Autopay Package and suspend your Cardholder Agreement during the Minimum Term, the Minimum Term (and your Autopay commitment) will be extended by the number of month(s) in the Proposed Suspension Period.
  7. If you have a Prepaid Package and suspend your Cardholder Agreement during the Prepaid Period, the Prepaid Period will be extended by the number of month(s) in the Proposed Suspension Period.
  8. You are not permitted to suspend your Cardholder Agreement if you are a Short-Term Cardholder.
  9. If you suspend your Cardholder Agreement for any reason, you will not be entitled to use the PURE Facilities and/or Equipment and/or to take part in any Activity during the suspension period, including access to PURECAST live-streamed classes unless you reactivate your Cardholder Agreement.
  10. If you reactivate your Cardholder Agreement before the end date of the Proposed Suspension Period:
    1. If you are on a Prepaid Package, the expiry date of your prepaid Agreement shall be adjusted if you request to end the suspension earlier.
    2. If you are on a Monthly Autopay Package, you agree to pay a pro-rata charge if you request to end the suspension earlier.
